 Causes of hair loss

It is important to establish the root cause of the problem because treating underlying conditions usually stops hair loss. In many cases you can do something about it at home such as lifestyle changes and a good diet. It is not always possible to establish the cause.

Likely conditions may be – imbalance, androgenic alopecia, thyroid disorders, after pregnancy, during menopause. Stress or emotional trauma, severe illness or surgery, liver or renal failure, crash diets /improper diets, iron and zinc deficiency. Medication: anticancer drugs, anticoagulants. Cholesterol lowering Infection: folliculitis, tinea, Herpes zoster trauma: burns, radiation, chemical or mechanical injury Inflammation caused by certain skin conditions Congenital aplasia

Aggravating Factors

Lack of sleep, jet lag, too much sun exposure, hair products, overstyling.

Hair Growth Cycle

Hair growth cycle consists of 3 stages:

Anagen – growth phase lasting years (85% of hair)

Catagenresting phase lasting 2 weeks (15 %of hair)

Telogenfalling out phase lasting 3 – 4 months.

Hair loss occurs gradually and is normally noticed when it is significant. First sign of increased hair loss is the amount of hair on the hairbrush or comb. An average person loses 100 to 150 hair daily which is normal. If there is a shock to the system or aggravating factors, up to 70 % of hair in growth phase (anagen) may go into falling out phase (telogen) in diffuse hair loss. Excessive hair loss should be investigated and treated to stop it getting worse. See your doctor to do the necessary tests to exclude an underlying illness.

is symmetrical, retraction of hair at the hairline with thinning of the central part of the scalp. Remaining hair is distributed in a crow-like pattern above the ears and above the neck. Hair becomes gradually thinner and slow growing. Paradoxically men experience an increased hair growth in ears, nose or eyebrows.

most commonly presents with diffuse thinning on top of head and crown, around the temples and at the hairline above the forehead. It may start as early as 30 years of age but is most noticeable in your 40s.

Androgenic alopecia (AGA) – is a genetically predetermined condition resulting in hair loss. The prevalence of AGA increases with age and is 31% in years of age going up to 53% at years. Androgenic alopecia is the most common cause of hair loss in men and women. The density of androgenic receptors is genetically predetermined however the mechanisms underlying AGA are not fully understood. Hair loss genes can be passed onto the children by either parent. Clue to the diagnosis is the hair pattern in parents and grandparents. Activation of the hair loss gene is determined by the levels of dihydrotestosterone (DHT) which is higher in the second and third decade of life and goes down fourth and fifth and becomes extremely low in the sixth decade of life. Higher levels of DHT results in faster hair loss in individuals with genetic predisposition for hair loss. Levels of DHT do not influence hair loss in individuals without genetically predetermined tendency to hair loss. DHT attaches to the hair follicles making them causing . In the hair follicles testosterone is converted into active metabolite called dihydrotestosterone (DHT) by enzyme 5 – alpha reductase and binds to androgenic receptors in hair follicles resulting in shortening of growth phase (anagen) and facilitating falling out phase (telogen). Gradually thick, pigmented terminal hair converts into thinner shorter hair to finally become short, wispy and vellus hair

sudden loss of large volume of hair common in women as the result of stress, traumatic life events, hormonal imbalance after of a baby or after serious surgery. It is often a “traumatic event” causing diffuse hair loss which will spontaneously without treatment after a few months.

Anagen effluvium

common during chemotherapy for cancer

Trichotillomania

obsessive hair pulling common in women and children with emotional issues.

autoimmune condition characterized by a sudden and near complete hair loss in one or more circular patches which is commoner in children and young adults. Short, blunt-ended hair tapered at the base (exclamation point hair) may be seen near the margin of the alopecia patches.

Grading determines whether treatment can be conservative or hair transplant. You will know that you are losing too much hair when hair is noticeably thinner, you notice receding or other people notice it. You may see excessive hair on your pillow, in the bath plug, in your hands or when styling hair.
